Full Name    (required)

The full name of the person to be sponsored is required It is recommended that the middle name or middle initial be given, if available. Please do not add a Jr., Sr., or any other suffix to the last name.

Date of Birth    (required)

This is the birth date of the applicant. You can enter this data in 'MM/DD/YYYY' format, using a 2- or 4-digit year. You can also enter the date without separators, as a 6 or 8 digit number. This field is required for the applicant to activate their UCInetID.

Social Security Number    (required)

This is the 9-digit Social Security Number of the applicant. This field will accept the social security number in the format '###-##-####'. Optionally, you may enter the social security number as a single, 9-digit number. This field is used to validate the applicant when the applicant activates their UCInetID.

Work Phone    (required)

This is the work phone number of the applicant. This field will be used to contact the applicant directly, if required. The 3-digit area code is required. You may enter the phone number using one of the following formats:

    (###)###-####
     ###-###-####

Optionally, you may enter the work phone number as a single, 10-digit number.

Email Delivery Address

If the applicant already has an email address that they would like to use to receive their email, you may enter it here. Otherwise, the applicant will be eligible for a NACS mailbox service account, the same as any faculty or staff member.

Title    (required)

Please give us a descriptive title for the Sponsored UCInetID holder. For example, 'Visiting Professor', 'Medical Researcher', or 'Volunteer'.

Sponsor UCInetID    (required)

This is the UCInetID for the sponsor of the applicant. All applicants for sponsored UCInetIDs must have a sponsor. The sponsor must be a UCI staff or faculty member, and will be verified as being one. Students cannot be sponsors.

Printed

Check this box if this applicant is to be included in the printed campus directory phonebook. It is uncommon to list sponsored UCInetID holders in the printed directory.

Expiration    (required)

Sponsored UCInetIDs are valid for one to a four 4 quarters at a time and may be renewed by the sponsor at any time throughout the year indefinitely. Sponsors and Sponsored UCInetID holders will be notified about upcoming expiration.
